  • If you need to request access to a report (or update your existing access to include departments), submit the Access Request Form. When submitting the access request, include the name of the report and any other details that might be neededthe departments you need to gather data about. When listing departments in your request, list the five digit department number or hierarchy role up (COHS = College of Health Sciences).
